What is HCS?
The Home and Community-based Services (HCS) Program is a Texas Medicaid waiver program designed to support individuals with an intellectual disability (ID) or a related condition. The program provides services and supports that can help individuals live meaningful, fulfilling lives in their homes and communities rather than in an institutional setting.
Who May Be Eligible for HCS?
The HCS Program may be available to eligible Texas residents with an intellectual or developmental disability (IDD) or a qualifying related condition who meet the program’s eligibility requirements.
A person may qualify for HCS if they:
-
Have an intellectual disability or qualifying related condition and meet the required level-of-care criteria.
-
Meet applicable intellectual functioning and adaptive behavior requirements, which may include an IQ of 69 or below or certain qualifying related conditions with an IQ of 75 or below.
-
Have deficits in adaptive behavior that affect everyday functioning and support needs.
-
Meet Medicaid financial eligibility requirements, which may include eligibility through Supplemental Security Income (SSI) or another qualifying Medicaid pathway.
-
Are not receiving services through another Medicaid waiver program. Individuals enrolled in another waiver may be required to choose which waiver program they wish to receive services through.
-
Choose HCS services instead of institutional ICF/IID services, allowing services and supports to be provided in a community-based setting.
Final eligibility and enrollment are determined by the appropriate Texas Medicaid and HHSC processes based on each individual’s circumstances.
Who Provides HCS Services?
The Texas Health and Human Services Commission (HHSC) contracts with approved HCS providers to deliver services and supports to individuals enrolled in the Home and Community-based Services (HCS) Program.
When enrolling in HCS, individuals and their families have the right to choose an HCS provider that serves their area. They may also choose to transfer to another HCS provider if their needs or preferences change.

As an approved HCS provider, Jonz 1 Forever Cares provides person-centered services designed to support each individual’s independence, choices, goals, safety, and participation in their community.
Interested in Receiving HCS Services?
Individuals who are not currently enrolled in HCS can contact their Local Intellectual and Developmental Disability Authority (LIDDA) to learn about eligibility and the process for requesting Medicaid waiver services. For individuals already offered HCS services, the LIDDA assists with the enrollment process and provides information about available HCS providers so the individual or legally authorized representative can make their own provider choice.
If you are currently receiving HCS services and are considering transferring providers, you also have the right to explore other HCS providers that serve your area.
Whether you're waiting for services, beginning the HCS enrollment process, or considering a provider transfer, Jonz 1 Forever Cares is available to answer questions about the services we provide and help you understand your options.

You do not necessarily have to wait for an HCS waiver opportunity to receive help at home.
Jonz 1 Forever Cares also provides non-medical home care through our Personal Assistance Services (PAS) program. Families who are waiting for HCS or another Medicaid waiver may be able to use our private-pay home care services for additional support in the meantime.
Depending on the individual's needs and care plan, non-medical support may include:
-
Personal care and assistance with daily routines
-
Bathing, dressing, grooming, and toileting assistance
-
Meal preparation and household support
-
Companionship and supervision
-
Respite and relief for family caregivers
-
Daily living and independence-building support
-
Community outings and participation
-
Mobility and transfer assistance
-
Support with routines at home and in the community

Our non-medical home care program is separate from HCS, so receiving private-pay home care does not mean an individual is enrolled in the HCS Medicaid waiver program.
