Home & Community-Based Services (HCS)
Jonz 1 Forever Cares provides person-centered Home and Community-Based Services for individuals with intellectual and developmental disabilities in Cypress and surrounding communities. Our focus is helping each person build independence, make meaningful choices, participate in their community, and receive support based on their individual needs and goals.
A Comfortable Place to Call Home
At Jonz 1 Forever Cares, we believe the quality of a residential environment plays an important role in an individual’s comfort, independence, and overall quality of life. Our HCS residential homes are thoughtfully maintained to provide a welcoming, comfortable, and respectful living environment.
Each residence is intended to feel like a home rather than a facility. Individuals are encouraged to personalize their space, participate in household routines, make choices about their daily lives, and remain actively involved in their communities. Our goal is to provide a residential setting where each person’s preferences, dignity, independence, and sense of belonging are respected.
We are committed to creating homes where individuals can establish routines, build meaningful relationships, pursue personal goals, and feel comfortable in the place they call home.
Jonz 1 Forever cares Hcs services
Residential Support Services (RSS)
Ongoing support in a three- or four-person residence, including personal care, daily routines, household activities, skill development, community participation, and support with greater independence.
Supervised Living (SL)
Residential support based on each individual’s needs, including assistance with daily living, personal routines, safety, independence, skill development, and community involvement.
Host Home / Companion Care (HH/CC)
A family-style residential option where an individual lives with a qualified host home/companion care provider and receives support with daily routines, personal care, household activities, independence, and community participation.
Supported Home Living (SHL)
Support for individuals living in their own home or with family. Services may include personal care, household tasks, daily living skills, community activities, and other authorized supports.
Respite Services
Temporary support that gives unpaid family caregivers time away from caregiving responsibilities while their loved one continues receiving appropriate assistance and supervision.
Nursing Services
Nursing services help address an individual’s health-related needs as authorized through their HCS service plan. Services may include nursing assessments, monitoring, medication-related support, health education, coordination of care, and other nursing services provided by qualified nursing professionals.

Not Sure Which Service You Need?
At Jonz 1 Forever Cares, we provide support through two separate programs, each designed to meet different needs. While both programs are operated by the same caring team, how you qualify and how services are paid for are different.
Non-Medical Home Care
Our non-medical home care program is available to individuals and families who need extra support with everyday life at home. This is primarily a private-pay service, meaning you do not have to be enrolled in the HCS Medicaid waiver program to receive care.
With home care, you already have a place to live — we bring the caregiver to you.Our caregivers can provide assistance with personal care, companionship, household tasks, respite, daily routines, community activities, and other approved non-medical support based on your needs.
This option may be helpful for seniors, adults with disabilities, individuals with IDD, people who need additional support at home, and families who simply need an extra helping hand.
Before services begin, we offer an in-home consultation to meet you and your family, learn about your daily routine, preferences, goals, and support needs, and determine whether Jonz 1 Forever Cares is the right fit for you. This also gives you the opportunity to ask questions, get to know our approach to care, and understand what the next steps would look like.
Home and Community-Based Services (HCS)
Our HCS program is a Texas Medicaid waiver program for eligible individuals with intellectual and developmental disabilities (IDD). Unlike private-pay home care, an individual must meet HCS eligibility requirements and receive an HCS waiver opportunity through the Medicaid interest-list process before enrolling in HCS services.
If you or your loved one is already enrolled in HCS, has received an HCS waiver opportunity, is going through enrollment, or is considering transferring HCS providers, our team can help you understand the next steps and learn more about the HCS services available through Jonz 1 Forever Cares.
